Job Details

We are seeking a full-time Case Manager to work in the Homeless Services Division of an established, nationally recognized provider. The Case Manager will ensure that participants receive the individualized services they need to help them successfully maintain housing and accomplish their recovery goals.

Pay Rate: $45,000 per year + Full Benefits
Location: Philadelphia, Pennsylvania
Work Arrangement: On-Site Position
Employment Type: Direct Hire 
Schedule: Monday-Friday 9:00am – 5:00pm

Responsibilities:

  • Providing engagement and case management support including home-based visitations and other services within the Philadelphia community. Approximately 75% of work is in the community

  • Interacting with members of the multidisciplinary team to ensure that participants receive the individualized services designed to help them successfully maintain their permanent housing

  • Conducting and coordinating outreach activities designed to locate and engage TCMU authorized participants

  • Providing clinical services to participants as needed in their natural environment or in their program housing or assist participants to access mainstream clinical services

  • Working with participants to develop, implement and monitor their personal goal plans to enable them to stabilize and retain housing on a permanent basis

Requirements: 

  • B.A/B.S. degree in Human Services/Social Science, Healthcare or a related field or equivalent experience

  • Proficient in Microsoft Office required

  • Experience in the behavioral health system preferred

  • Experience working in healthcare databases preferred

  • Empathetic individual with strong active listening skills 

  • Must posses a valid driver’s license

  • Must have reliable transportation
