Case Manager Needed to Support a Behavioral Healthcare Institute in Connecticut

A behavioral healthcare organization dedicated to delivering exceptional behavioral healthcare services, integrated medical, psychological and traditional self-help approaches to addiction treatment is seeking a Case Manager. The Case Manager provides routine case management services by coordinating the medical, clinical, social, and related needs of clients during treatment. This role involves maintaining contact with clients, their families, and service providers while acting as a member of a multi-disciplinary treatment team.

What Your Day Looks Like

  • Collaborate with the Clinical Director, Medical Team, Primary Clinician, Case Managers, and other treatment team members to plan and implement partial hospital treatment programs.
  • Assess clients' service needs to ensure appropriate support throughout treatment.
  • Ability to understand and respond to the emotional and social needs of clients and their families with sensitivity and care.
  • Effective verbal and written communication to interact with diverse clients, families, and interdisciplinary team members.
  • Develop and implement discharge plans and arrange for ancillary services during and after discharge.
  • Conduct continuing stay authorization reviews with insurance and managed care companies as needed.
  • Interview clients to gather information for service needs and create individualized treatment plans.
  • Manage a caseload of clients, providing ongoing case management, service referrals, and follow-up care.
  • Facilitate the education components of programs under the supervision of the Primary Clinician.
  • Collect urine samples for routine and random drug testing.
  • Act as a liaison between clients, families, and aftercare agencies.
  • Identify problem areas, service gaps, and barriers to services, making recommendations for improvements.
  • Document client records in compliance with all regulatory and program requirements.
  • Assist management by providing input on policy and procedure changes.
  • Participate in meetings, conferences, and continuing education programs to maintain knowledge and skills.
  • Perform other related duties as assigned.

What You Bring:

  • Bachelor’s Degree in Social Work or a related field (relevant education and experience may substitute for one another).
  • Experience in social work or a related field.
  • Strong ability to interact effectively with members of the healthcare system and patient population.
  • Capability to address client needs within the organization and in external settings.
  • Requires sitting, standing, walking, climbing, and bending.

Work Environment

  • Primarily an indoor office and patient care environment, with protection from weather but not necessarily temperature changes.
  • Potential exposure to communicable diseases.
