A behavioral health provider offering a range of comprehensive services, including mental health support, substance abuse treatment, wellness, and recovery programs, is seeking an Access Care Manager II to join the team.

The Access Care Manager II is responsible for assessing prospective clients for medical necessity and ensuring their placement into the appropriate program. This role involves conducting thorough and detailed biopsychosocial assessments for a diverse clientele spanning different social, ethnic, and age backgrounds. Additional responsibilities include performing hospitalization screenings and continued stay reviews in a professional, friendly, and non-judgmental manner.

What Your Day Looks Like:

  • Conduct comprehensive biopsychosocial assessments to determine appropriate program placement.
  • Maintain a welcoming attitude towards all clients, particularly those with co-occurring substance use disorders.
  • Screen clients for hospitalization needs and perform continued stay reviews as necessary.
  • Coordinate with internal teams and external providers to ensure seamless client care.
  • Communicate effectively with clients, families, and healthcare professionals to facilitate treatment planning.
  • Maintain and safeguards health information in compliance with applicable regulations.
  • Complete documentation and required paperwork by the end of each workday.
  • Provide guidance on evidence-based practices and appropriate treatment modalities.
  • Ensure timely and accurate documentation of client interactions, assessments, and referrals.
  • Monitor client progress and makes necessary adjustments to care plans.
  • Other duties as assigned  

 

What You Bring:

  • Minimum: Licensed Master Clinician (LLMSW, LMSW, LLPC, LPC, TLLP, LLP).
  • Fully Licensed Master Social Worker (LMSW) preferred.
  • Demonstrate cultural competence and recovery-based practices.
  • Utilize trauma-informed care approaches.
  • Understands medical necessity requirements and Medicaid regulations.
  • Demonstrate competency in differential diagnosis.
  • Maintain knowledge of various service requirements.
  • Possess extensive knowledge of the DSM and diagnostic criteria.
  • Exhibit exceptional writing and documentation skills.
  • Collaborate effectively with different departments and supervisors.
  • Remain knowledgeable about community resources.
  • Facilitate coordination between inpatient and outpatient services.
  • Utilize basic counseling and psychotherapy techniques effectively.

  Physical Requirements:

  • Ability to stoop, bend, and lift up to twenty-five pounds.
  • Manual and digital dexterity required for case documentation, telephone operation, and keyboard use.
  • Capable of working in a high-stress environment with productivity expectations.
  • Potential exposure to infectious disease processes.
