As the Speech Therapist (PRN), you will provide evaluation, treatment, and consultation services to individuals with communication, cognitive, and swallowing disorders across skilled nursing, outpatient, and home health settings. You will develop and implement individualized care plans, collaborate with interdisciplinary teams, and educate patients and caregivers to support continued progress. This role is responsible for delivering high-quality, compliant therapy services while maintaining accurate documentation and adhering to all regulatory and professional standards.
Essential Duties and Responsibilities

Conduct evaluations of speech, language, cognitive, and swallowing function.

Develop and adjust individualized treatment plans based on patient needs and progress.

Set measurable goals and track patient outcomes.

Provide therapy to improve communication and swallowing abilities.

Educate patients, families, and caregivers on techniques and home programs.

Collaborate with the care team to ensure coordinated services.

Maintain accurate and timely clinical documentation.

Participate in team meetings, training, and quality initiatives.

Maintain licensure and stay current with best practices.

Ensure compliance with all regulatory and privacy standards.

Qualifications
-
Strong verbal and written communication skills in English.

Master’s degree in Speech-Language Pathology or a related field from an accredited program.

Active and valid Speech-Language Pathologist (SLP) license.

Certificate of Clinical Competence (CCC-SLP) from ASHA.

Current CPR certification.
-
Meets all applicable licensure requirements.

Valid driver’s license and proof of auto insurance.

Ability to remain on duty during emergencies or disaster situations, as needed.

Prior experience in speech therapy preferred (minimum of one year).
